Why does a red "New" box show in chat?
For some reasons, a red box saying "New" with a dashed line under it shows after a short time in the visitor's chat box? This is annoying, how do we avoid this?

Im not sure you can avoid.
Try to see in widget settings.

What causes it? It only recently started happening, can't figure out why. I didn't change the widget settings.

Maybe you upgraded live helper chat?
In any case you can edit text and color in widget theme setting under the message style tab.
You can try to see if removing the text will avoid the new message label to be showed also maybe try to change color 
Or maybe you can hide with some CSS if this previous steps doesn't helps.

This feature can't be turned off but you can change it's style using Widgtet theme
Widget themes -> Messages style

The new message label is showed if the visitor or the operator receive a message but is not active on the page to see it so this indicator help the user or the operator to find new messages while them was inactive or away
