client is not getting message.

I start a chat session by going to abc.com/chat/index.php and that works good and the message gets sent to an operator using with wwindows app and or the web interface. but at the top to the window of the person that started the chat there is a message "Pending confirm"

so I have three users
1. the person that started the chat
2. an operator using the windows client
3. an operator (admin) using the web dashboard.

both operators have joined the chat

client 2 can see what client 1 and 3 type
client 3 can not see what is typed but if close chat and reopen then what was typed shows up.
client 1 can only see what they type.

This had been setup and working with out a problem and it seems like it broke over the weekend and I'm not sure where to look.

It seems like the web clients are not getting the messages as they are typed and sent.
