offline form looks ugly

After user is redirected to the offline form... he has this screwed form looking.

Then there is this message where i cannot find anywhere>
There are no online operators at the moment, please leave your message

how can i make the form looks like the online chat form...?

i am using the latest version from Saturday

Hi, this is a know issue, please see: https://www.peopleinside.it/liveticket/kb/faq.php?id=2

The issue is the number 1 and the link will redirect you to the GitHub issue where you can also understand how to FIX until the chat owner will not fix this BUG.
