need help with chat integration

hi,

I am new with this software i would like to know how can i add more fields in chat starting form as a contact form like given below

name,email,question, details etc.

i want to add these more fields in chat also when user submit these information i have a saperate database where i want them to be added i am using this information somewhere else i would like to submit all these inserted data in another system with is not linked but i need this chat startup information to be processed all the chat will be working same way it work but i need the information to be inserted in other saperate table

please help

maybe for this you need commercial support as seems to be a customization
Maybe.
