Asking User about his details?

Is there any possibility in Live helper chat application to ask user his Name and mail id, instead of passing the values as LHCChatOptions.attr_prefill.push({'name':'username','value': 'Test' });?

As showed in the below image (I want only Username and Email)

Yes just configure form start fields.
