Well I have a operator and:
I followed this steps:
System configuration > Users > Edit some operator user > Permission > [lhosticket] osTicket > check [use] Allow operator to create an issue from back office > Press Request permission > (after modal windows) press Request permission again .
ERROR is "Please choose a user!"

You are doing it at wrong. Place permission tab just shows permission does not manage them. You have to use Roles editing interface.
