How to ask for email on chatbox

Hi im just beginning with chatbox and create one to test at http://discabos.com.br/obrigado-por-nos-contatar/
I need that customers fill name and email to start on this chat but it only asks for name.
How can i do that?

Simply Go in admin panel > setting icon on the top  > Live help configuration > Start chat form settings > online form settings
Here is Email section, just check on these options
This field is visible in the popup
This field is visible in the page widget.
Now clean your cache and enjoy

Some misunderstandings here

1. "tizimazza" you have embeded chatbox, not live help widget. This widget is plain shoutbox it does not have any feature like mail request etc.
2. As for "Zeeshan" yes that's the correct way to do if you have embeded live help widget.
