Survey after close chat

Hi, may I know what are the possible factors or config that will affects survey not showing up after operator close chat or visitor close chat?

Your version - 3.82

Done the following but still not appearing, the screen stuck at "A support staff member has closed this chat":
Configured survey list and put under default department.
Configures Survey at the end of chat under Widget theme.

Are you sure to have included the survey when you generated the embed code?
When you generate the embed code you need select to show the survey.

Can you try to generate a new embed code selecting the survey?
