Idea for a new embed module, need an opinion

Hi all,
Basically my idea is have an module which would allow to have.

1. Flexible number of custom fields.
2. Module would allow to embed form in page or just provide client a link to fill a form.
3. Form content would be filled using some bbcode, for example visit at doctor form example

[TEXTAREA START]Please fill your visit deatails
Name
{type_text_name_name}
Surname{type_text_name_surname}

Visit purpose{type_textarea_name_purpose}
.....[TEXTAREA END]
Basically this module would be usefull for small offices which offers for clients something like
1. Registration to courses
2. Registration to visits at doctors
3. Need general custom forms
4. Issues registration forms.And so on, just everywhere where custom/complex feedback is required.
Any ideas, would this module be useful or not?
